Case Manager for Developmental Services YSSN

Join Your Support Services Network as a Case Manager devoted to developmental services in Aurora, Ontario. Provide essential support and advocacy for individuals with diverse mental health needs.

This role offers two full-time contracts focused on case management for children and adults with developmental disabilities. You will assess client needs while developing and coordinating personalized service plans that promote independence and enhance quality of life. Engaging clients with their community and navigating available resources will be a key part of your work.

Key Responsibilities:

• Perform thorough assessments to determine client needs

• Create and manage tailored service plans for individuals

• Facilitate and coordinate necessary community referrals

• Assist clients in building independent living skills

• Aid clients in participating fully in their communities

Requirements:

• Bachelor’s degree in a relevant field like social work

• At least three years of relevant case management experience

• Experience with crisis intervention and problem-solving

• Strong communication skills and software proficiency

• Access to a reliable vehicle for travel
