As part of our Behavioural Support Services Team, your mission is to enhance the quality of life for clients experiencing mental health and neurological challenges. This role involves direct client support, collaborating with families, and coordinating essential behavior management strategies to ensure safe transitions and service continuity.
Key Responsibilities:
• Implement individualized Behavioural Support Plans
• Conduct timely behavioural assessments and documentation
• Advocate for clients while collaborating with care teams
• Provide training and counseling for caregivers
• Develop crisis management strategies and interventions
Requirements:
• Relevant post-secondary education
• Minimum 3 years’ experience in a similar role
• Knowledge of community resources and treatment approaches
• Proven crisis intervention and problem-solving skills
• Proficient in documentation and basic computer tools
Foster client-centered support and behavioral interventions to improve seniors' lives within a collaborative care environment.
