Job Overview

The Clinical Pharmacist Specialist ensures safe, appropriate, and economical use of drugs through the application of specialized skills and knowledge in pediatric patient care.

This pharmacist will help support our Bone Marrow Transplant (BMT) team to provide care to our growing BMT population. The position will rotate with the other two (2) pharmacists between ambulatory and inpatient BMT services.

The Department of Pharmacy provides comprehensive clinical services to patients at Children's Hospital Colorado. We have organized clinical pharmacy services that complement the team approach to providing care. Our clinical pediatric pharmacists are available 24 hours a day and help ensure the safe and effective use of medication in children - as well as promote health, wellness and disease prevention. Our specially-trained pharmacists work with physicians and other healthcare professionals to determine the best medication regimen and monitoring plan for the pediatric patient.

Clinical pharmacy services are integrated into virtually all inpatient care units as designated pharmacists round with the medical teams. Through this direct involvement, pharmacists are empowered to assist in a great deal of therapeutic decision making and therapeutic drug monitoring activities.

Shift: Monday-Friday. Every 3rd weekend rotating and selected holidays.

Duties & Responsibilities
  • Designs and leads pharmaceutical outcomes research. Identifies opportunities for and participates in drug utilization reviews and research. Identifies opportunities for publishing case reports, unique therapeutic interventions, development of practice standards, and other medication related pharmacy topics.
  • Participates in the education of pharmacy staff, nursing staff, medical staff, pharmacy residents and students, other healthcare allied professionals and patients and families. Serves as a preceptor and mentor for pharmacy residents and students.
  • Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner; using a systematic approach to effectively searching, retrieving, and evaluating the literature and appropriately communicating and applying the information to the patient care situation; and assisting with the development, maintenance, and implementation of hospital and department policy and procedures related to medication use, storage, restrictions, and administration.
  • Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS). Demonstrates sustained record of identifying and leading quality improvement/assurance initiatives.
  • Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges through participation in multi-disciplinary team rounds and/or care, emergency response, education, guidance, interpretation and analysis of medication related therapeutic issues; documenting pharmaceutical interventions according to departmental standards; performing therapeutic drug monitoring and disease outcome analysis; and timely verification of medication orders in an electronic health record system.
  • Participate in general pharmacy operations throughout the organization through appropriate medication preparation, dispensing, and inventory control. Provides oversight to pharmacy technicians, interns, residents and students as needed.
Minimum Qualifications
  • Education: Doctor of Pharmacy or Bachelor of Science in Pharmacy degree
  • Experience: Two (2) years of residency experience or one (1) year of residency plus three (3) years of work experience
