Full‑time Assistant – responsible for planning, implementing, and evaluating the Connections activity program for residents in Assisted Living, Memory Care, or Skilled Nursing settings.

• Enter resident information in multiple systems.
• Attend care plan meetings with families and residents.
• Collect resident profiles.
• Assist in planning, developing, organizing, implementing, and evaluating the Connections activity program for the community.

• High School Diploma. Preferred Bachelor’s Degree in Social Work, Therapeutic Recreation, Psychology, Gerontology, or related field.
• Minimum 21 years old with a valid state driver’s license. Preferred previous experience in healthcare or service related setting.
• Knowledge of physiology and psychology of older adult.
• Certification in CPR and First Aid.
• Must have weekend availability.
