Job Title: Peer Recovery Specialist - Medication Mobile Unit

Description

This full-time position is supported by grant funds, whose funding will be reviewed on a periodic basis. This position is contingent on grant funding and is time-limited. The position''s status and viability may be affected over time by any applicable variations in the grant and/or funding amounts.

Key Responsibilities
  • Serve as a Peer Specialist and educator to implement interventions to improve outcomes for patients who enter care via the ARTS Medication Mobile Unit and/or ARTS Clinics.
  • Identify problems, challenges, or obstacles in coordination of services for patients, and in a timely and effective manner, ensuring all duties and responsibilities are performed for the benefit of patients receiving care.
  • Conduct structured, Peer-led individual and group interventions focusing on health education, care coordination, identifying and reducing barriers to care, and conduct outreach activities to engage pregnant and postpartum patients in SUD treatment.
  • Conduct case management for patients, provide referrals to resources, implement standardized interventions, track engagement, and connect patients to prenatal treatment and care.
  • Collaborate with supervisor and staff, coordinate efforts to ensure service delivery, attend trainings and meetings, participate in project advisory committee meetings, and travel as needed for care coordination; ensure confidentiality of patient information.
  • Maintain accurate records and notes as required, use electronic health record systems for documentation, schedule and track appointments, and collect and report patient data.
  • Work with patients referred from parole/probation, social services, and other agencies, and perform other duties as assigned by supervisor and/or management.

Qualifications

Minimum Qualifications
  • High school diploma or GED.
  • One (1) year of experience working closely with patients and connecting them to resources.
  • Additional appropriate education may substitute for the required experience on a year-for-year basis.
Preferred Qualifications
  • Associate's degree or higher in psychology, social work, counseling, human services, or related field.
  • Certification as a Peer Specialist (e.g., Colorado Peer and Family Specialist/CPFS).
  • Current, valid Colorado DORA registration as Certified Addiction Technician (CAT) or higher.
  • Experience with motivational interviewing; patient navigation; electronic health records; working with clients involved with the criminal justice system; and providing administrative support in a substance abuse treatment setting.
  • Experience managing financial paperwork and gathering statistics; sexual health education and/or HIV experience; bilingual English/Spanish.
Knowledge, Skills, and Abilities
  • Excellent interpersonal, written, and oral communication skills.
  • Proficiency with Microsoft Office (Excel, Outlook, Word) and data entry; ability to maintain accuracy and timelines; strong multitasking and critical thinking skills.
  • Ability to establish rapport, maintain boundaries, and exercise discretion to protect patient confidentiality.
  • Knowledge of care coordination and community resources; ability to engage with substance-dependent clients in treatment.
Conditions of Employment
  • Complete CCAR Recovery Coach Academy training within 30 days of hire.
  • Obtain CPFS certification within 4 months of hire.
  • Must possess a valid driver''s license and automobile insurance; willingness to drive as part of duties; ability to travel.
  • Background checks required (CBI and Trails) and fingerprinting; pre-employment and ongoing drug/alcohol testing per policy.
  • Must be able to work some weeknights, Saturdays, and Sundays as needed; may be on-call for emergencies.
  • Must have personal, lived experience with SUD and maintain continuous recovery with supports.
  • Must be able to work with patients referred from various agencies.
