Licensed Clinical Social Worker

Our Skilled Nursing Facility is seeking a highly skilled and compassionate Regional Licensed Clinical Social Worker LCSW to join our team in Aurora area. As a Licensed Clinical Social Worker, you will be responsible for providing high-quality mental health services to our clients, including assessments, therapy sessions, and case management. • Provide clinical services to clients, including assessments, individual and group therapy sessions, and case management
• Develop and implement individualized treatment plans in collaboration with clients and their families
• Conduct many other duties, including mental health evaluations, crisis intervention, and referrals to community resources
• Collaborate with multidisciplinary treatment teams to ensure comprehensive care
• Maintain accurate and timely records of client services and treatment plans
• Participate in quality improvement initiatives and contribute to the development of program policies and procedures

• Must be a Licensed Clinical Social Worker (LCSW) in the state of Colorado
• Minimum 2 years of experience in a clinical social work setting
• Master's degree in Social Work or related field
• Possess excellent communication and interpersonal skills
• Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
• Be familiar with the geriatric or aging population
• Have a current NPI number or a willingness to obtain
