5-star Patient Survey Rating on Medicare Care Compare | Voted Best Home Health Agency 2026 by Elder Review | A+ rated by the BBB
Avida Home Health is a Medicare-certified, ACHC-accredited agency based in Greenwood Village. We serve patients across the South Denver metro and we're known for complex wound care. We're adding a Physical Therapist to cover Aurora and the east metro.
You'd work with patients recovering from surgery, hospital stays, falls and chronic conditions, many of them dealing with wounds that affect mobility. Our clinical team is small and works closely together, so you'll know your DON, your PT Lead and your schedulers by name.
- Routine treatment visits: $85-$95
- Evaluations and SOC visits: up to $120
- Part-time or full-time
- No minimum number of visits per day
- You choose how many visits you take each week and we build your caseload around it
- Do PT evaluations, reassessments and treatment visits in patients' homes
- Complete OASIS assessments when PT opens the case
- Write functional, measurable goals and update plans of care
- Coordinate with nursing on wound care patients (positioning, offloading, mobility)
- Document in Alora at the point of care
- Home health experience preferred. New grads with strong clinicals are welcome to apply.
- OASIS experience is a plus. We'll train you if you don't have it.
- Valid driver's license, reliable vehicle and auto insurance
- CPR/BLS
- Patients clustered in Aurora and the east metro, so you spend less time driving
- A PT Lead you can call, not a regional manager three states away
- Point-of-care documentation, so you're not charting at 9pm
$85-$120 per visit, depending on visit type and experience
Benefits:- Mileage pay for travel between patient visits
