Case Manager (Bilingual, English/Spanish) at Family Focus — IWC Case Manager (Bilingual, English/Spanish) position in Aurora, IL. Family Focus is your partner in building strong families across Chicago and Northeast Illinois. We support families and their children including emergency assistance, immigration programs, child welfare services, family support services, educational programs, and more. Our goal is to help our community thrive and reach its full potential.

Family Focus operates 11 centers around Chicagoland and is expanding its reach to serve more families. We are seeking passionate individuals who share our vision to join our team as we grow.

• Conduct case management/family support duties including needs and strengths assessments, screenings, referrals and advocacy.
• Perform case management/family support home visits and follow-ups as needed.
• Locate, identify and utilize partner agencies, social services and resources for families.
• Provide education, information and referrals to program participants.
• Identify partners, speakers and curriculum to meet participant needs and provide education series for parents on topics such as parent-child interaction, literacy and language acquisition, immigration services, and more.
• Develop an Individual Family Support Plan for each participant receiving case management/family support services.
• Collaborate with local social service agencies to coordinate services for participants.
• Represent Family Focus Aurora in community councils, coalitions, partnerships, and other meetings as assigned.
• Attend required meetings, including weekly intake meetings, participant staffing, team meetings, program meetings and supervision.
• Prepare and submit documentation and reports; maintain participant records in accordance with professional standards and funding requirements.
• Develop and maintain required records, reports and statistical data.
• Perform other duties as assigned.

• Bachelor’s degree in Human Services, Social Work, Child Development, or related field.
• Two years of experience working in a strength-based environment with families, children, and youth.
• Prior case management/family support experience.
• Experience applying the Principles of Family Support Practice.
• Experience in a community-based/social services setting; ability to work in diverse communities.
• Culturally competent and understanding of the immigrant and refugee community in Aurora and surrounding areas.
• Must be able to write, speak, facilitate, and translate in both English and Spanish.
• Some weekends and evenings required; some travel with reliable transportation.

• Salary: $50,000/year
