Description

Midwest Asian Health Association (MAHA) is a non-profit organization with a mission to reduce health disparities for medically underserved populations through culturally sensitive and language appropriate community health programs. MAHA is seeking a program coordinator for its suburban office at: 4360 E New York St. Room# 105, Aurora, IL 60504 (Pacifica Square).

The Program Coordinator will be responsible for community health initiatives including immunization drives, health education workshops, and outreach. The Program Coordinator will also provide ACA Navigator Services (primarily from November through January 15th for open enrollment, and throughout the year for special enrollments) by reaching low-income and/or uninsured clients, and scheduling appointments for enrollment and post-enrollment services. The coordinator is highly visible in the community and will be a prime representative of MAHA in the Western Suburbs.

This job is also well-suited for a transition into Project Management, based on productivity and initiative shown during the probationary period and beyond.

Description of Duties/

Essential Functions:

  • Plan and implement immunization and community health program activities and events in DuPage and Kane counties and surrounding communities.
  • Recruit program participants and ensure the healthcare service needs and program goals are met according to the established schedule.
  • Develop partnerships with community organizations, schools, health care providers local public health departments, health and human services departments, and other social service agencies and organizations throughout the region.
  • Identify the community leaders to gain their input and ideas for immunization, education, outreach, and program development.
  • Organize program related partnership meetings and activities, coordinate immunization events and health education workshops, and conduct outreach activities with communities, schools, churches, community-based organizations, and other partners.
  • Implement intervention plans and strategies developed through community needs assessment.
  • Develop health education and outreach materials, and evaluation tools.
  • Conduct referral services and provide proper follow ups.
  • Document program and compile progress reports in a timely manner.
  • Take responsibility as the primary liaison between MAHA’s suburban office and main office and willing to travel to the main office in Chicago for training in order to perform independently as needed.
  • Conducting in-depth interviews with applicants to help determine eligibility for health insurance.
  • Provide one-on-one application assistance for health insurance or various benefits at partner sites, community sites, health and enrollment fairs, and any other opportunity identified by partners to reach potential enrollees.
  • Provide information and assistance in a fair, accurate and impartial manner that is culturally and linguistically appropriate to diverse communities.
  • Any other administrative and clerical duties as they arise.
  • Other duties as assigned.

Experience and Qualifications:

  • Two years experience in a consumer-facing public/community health role is required.
  • Ability to work with diverse populations, providing direct, face-to-face customer service; maintain high level of sensitivity to those populations; maintain privacy and security standards.
  • Proficiency in EITHER English and Chinese verbal communication skills OR English and Spanish written/verbal communication is required.
  • Able to drive with a valid Illinois driver license and auto insurance
  • Proficiency in Microsoft Office and office equipment
  • Knowledge or experience with health insurance eligibility and enrollment is preferred
  • Knowledge of public benefit systems is preferred
