Community Health Partnership of Illinois is a non-profit organization that provides primary health care to migrant farm workers and other medically underserved populations throughout northern and central Illinois. The organization specializes in primary preventative medical, dental, and behavioral health services. Our focus is delivering quality, culturally responsive health care services to our patients, caregivers, and partners in the communities we serve.
The Outreach Nurse provides primary medical services and completes progress notes and assessments in the electronic health record in a timely manner. Provide services to consumers based on current risk and status. Work with external stakeholders – such as hospitals, farms, public schools, shelters, jails, and primary care – to achieve positive client outcomes.
Description Of Primary Responsibilities
Responsibilities- The ability to meet and engage vulnerable people in various settings and life circumstances.
- Sensitivity to emotional cues of others; respond appropriately; interview in a non-threatening manner.
- Inclusive, positive attitude toward vulnerable people.
- RN degree from an accredited school of nursing
- Advanced nursing degree from an accredited school or college of nursing
- Current License as a registered nurse, Illinois
- Advanced practice registered nurse (APRN) license, Illinois
- Approved APRN board certification
- BLS Certification
- DEA License
- Not Applicable
