Family Tree Rehabilitation, part of The Campus of Anna Maria, is seeking a PRN Physical Therapist (PT) to support our therapy team as needed. Situated on a beautiful wooded campus in Ohio, we are a family‑owned and operated skilled nursing facility dedicated to providing high‑quality, resident‑centered rehabilitation services in a collaborative and supportive environment.
Position SummaryThe PRN Physical Therapist is responsible for evaluating residents, developing and implementing individualized physical therapy plans of care, and helping residents improve mobility, strength, balance, and overall functional independence.

- Perform physical therapy evaluations and establish individualized plans of care
- Provide skilled physical therapy treatment services in accordance with physician orders
- Collaborate with and supervise PTAs as applicable
- Accurately document evaluations, treatments, progress notes, and discharge planning
- Participate in interdisciplinary care planning and team communication
- Ensure compliance with facility policies, professional standards, and regulatory requirements

- Graduate of an accredited Physical Therapy program
