Registered Nurse; RN Acute Care – Care Coordination Sr.; Day Shift

We are seeking a Care Coordinator RN Senior to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure comprehensive care during complex hospital admissions.

• Conducting psychosocial, functional, and discharge assessments
• Collaborating with physicians, care teams, and social services to develop individualized discharge plans
• Managing all aspects of discharge planning and addressing barriers to timely discharge
• Facilitating communication among care team members to promote efficient care progression
• Documenting assessments, care plans, and interactions accurately in medical records
• Mentoring and precepting new staff and interns
• Supporting professional development and complying with policies and standards

• Minimum 3 years of acute care nursing experience and 2 years in hospital-based care coordination or case management
• Associates or Diploma in Nursing;
  Bachelor’s preferred
• Licenses and certifications: RN licensure, BLS certification
• Strong communication, assessment, organizational, and problem-solving skills
• Knowledge of healthcare regulations and community resources
