Qualifications:

• Licensure:
  Must hold a current Doctor's license from the State of Texas Medical Board
• Residency:
  Successful completion of a residency program in Internal Medicine (IM) or Family Medicine (FM)
• Certifications:
  • Current Drug Enforcement Agency (DEA) registration certificate
