Overview
The Texas Home Living (TxHmL), General Revenue (GR), and Community First Choice (CFC) programs provide essential services and supports so that people with IDD can continue to live with their families or in their own homes. This program serves individuals diagnosed with Intellectual and Developmental Disabilities in Travis County.
Integral Care provides service coordination for the programs listed above, which includes:
- Robust assessment to support treatment planning (i.e. person directed planning).
- Coordination of services including the identification of service gaps and referral needs.
- Ongoing monitoring of services to ensure they are occurring, are helping the individual progress towards their identified goals, and that the individual is satisfied with their services.
- Crisis management through the coordination of services to support individuals with IDD remain safely and happily in the community.
- Support individuals with IDD through comprehensive service coordination as part of the TxHmL, GR, and CFC programs.
- Conduct robust assessments to inform treatment planning and person-directed planning.
- Identify service gaps and coordinate referrals to ensure continuity and quality of care.
- Monitor services to confirm they are delivered, contribute toward individual goals, and meet the individual’s satisfaction.
- Coordinate crisis management to help individuals remain safely in the community.
Education
Required:
Graduated from an accredited college or university with a Bachelor’s degree with a major specialization in Human/Behavioral/Social Services or related field.
License
Required:
Current and valid Texas driver’s license and a clean driving record.
Center certification to perform service coordination is required (training will be provided during onboarding to obtain this certification).
Experience
Preferred:
Bilingual.
Hours: As needed.
Salary: $24.18- $25.39 an hour based on education.
