Physical Therapists are r esponsible for supporting patient care and supporting clinic success by:

• Synthesizing pertinent information from a patient's referral, medical records, imaging, intake forms and subjective history taking.
• Diagnosing movement dysfunction via functional movement, posture and gait analysis, internal pelvic floor examinations, objective measurements and listening to patients concerns and goals.
• Using critical thinking to develop individualized plans of care for patients to include the patient's goals and expected outcomes of the plan.
• Utilizing evidence‑based interventions such as therapeutic exercise, manual therapy, neuromuscular re‑education, biofeedback and other modalities to achieve patients' goals.
• Evaluating and documenting patient progress, utilizing outcomes to drive results and modifying the plan of care as needed.
• Educating patients about what to expect from the recovery process and how to cope with challenges throughout the process
• Following up with patients to ensure better adherence to plan of care
• Connecting with providers and local organizations to increase awareness of therapies provided by the clinic and ensure that as many patients as possible that need care…
