Position OverviewThe Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the care plan and physicians’ orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs; report changes to supervisor and/or physician as appropriate.
- Document all activities, assessments, nursing actions, responses, and coordination of care timely when care is delivered.
- Participate, implement, and update the nursing care plan.
- Takes appropriate nursing action based on assessment and achieves expected outcomes.
- Recognize changes in patient needs and responses requiring intervention; implement care to prevent or reduce risk.
- Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Apply infection control measures protecting staff and patient per OSHA standards.
- Educate patient and family regarding disease process, self‑care techniques, and prevention strategies.
- Maintain knowledge of nursing competencies through educational programs, continuous education units, internal learning management, and skill evaluations.
- Active, unencumbered license (LPN/LVN) in the state of practice; compact licenses transferred within 90 days.
- Current CPR certification (hands‑on component). Aveanna can assist in obtaining if necessary.
- TB skin test current within last 12 months.
- Six months prior hands‑on nursing experience preferred but not required.
- Reliable transportation.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as an LPN/LVN.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.
- South Carolina – 1 year of pediatrics experience.
- California – 1 year of experience under current nursing license.
- Louisiana – 1 year of experience as a licensed nurse.
- Continuing education as required by state.
- Six months of recent experience as an LPN/LVN in a clinical care setting.
- Home health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Good organization and communication skills.
- Speak, write, read, and understand English.
- Travel capability.
- Lift 50 pounds.
- Reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Sit and climb stairs.
- Visual and hearing acuity.
- Strong sense of smell and touch.
- Respond physically and mentally to emergency situations in the home or during transport.
- Function in a wide variety of environments involving exposure to allergens and other conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
