Position Overview

The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the care plan and physicians’ orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

Essential Job Functions

• Deliver and coordinate quality patient care in compliance with physician orders.
• Continuously observe and assess patient condition and care needs; report changes to supervisor and/or physician as appropriate.
• Document all activities, assessments, nursing actions, responses, and coordination of care timely when care is delivered.
• Participate, implement, and update the nursing care plan.
• Takes appropriate nursing action based on assessment and achieves expected outcomes.
• Recognize changes in patient needs and responses requiring intervention; implement care to prevent or reduce risk.
• Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
• Apply infection control measures protecting staff and patient per OSHA standards.
• Educate patient and family regarding disease process, self‑care techniques, and prevention strategies.
• Maintain knowledge of nursing competencies through educational programs, continuous education units, internal learning management, and skill evaluations.

Qualifications

• Active, unencumbered license (LPN/LVN) in the state of practice; compact licenses transferred within 90 days.
• Current CPR certification (hands‑on component). Aveanna can assist in obtaining if necessary.
• TB skin test current within last 12 months.
• Six months prior hands‑on nursing experience preferred but not required.
• Reliable transportation.

Requirements

• Graduate of an accredited school of nursing.
• Current, unrestricted state license as an LPN/LVN.
• Current CPR certification.
• Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.

Additional State‑Specific Requirements

• South Carolina – 1 year of pediatrics experience.
• California – 1 year of experience under current nursing license.
• Louisiana – 1 year of experience as a licensed nurse.
• Continuing education as required by state.

Preferences

• Six months of recent experience as an LPN/LVN in a clinical care setting.
• Home health experience.

Environment

• Function in a wide variety of environments involving exposure to allergens and other conditions.
• Possible exposure to blood, bodily fluids, and infectious diseases.
