You will perform admissions visits, conduct assessments, develop plans of care, manage symptoms, and educate patients, families, and facility staff on hospice care.
The RN will supervise CNAs/LVNs/homemakers, coordinate with physicians and the interdisciplinary team, document accurately, and support discharge planning.
