Life Works of Austin, TX is looking for a Case Manager to join our Rapid Rehousing team! A Case Manager provides case management services to clients, draws upon clients’ strengths to inform service planning, advocates on behalf of clients, collaborates with other service providers, and manages crisis situations. This position provides administrative support to programs by maintaining client files, entering data, and completing required documentation in compliance with multiple contracts.

Additional duties include intake, service planning, rapport building, monitor client’s progress, group facilitation, and providing resource referrals relevant to the client’s action plan.

This full time, non-exempt position earns a competitive rate starting at $24.86/hr, and benefits; 24 PTO days per year (accrued); 11 paid holidays per year; a premium-free employee medical plan with $100 per month HSA contribution (plus other affordable employee or family medical, dental, and vision options!); a retirement savings plan with 50% match on the first 6% of contributions, and much more!

RESPONSIBILITIES

  • Provide client-centered, trauma-informed case management services to diverse individuals that have experienced chronic homelessness, diagnosed mental health illnesses, substance use disorders, a history of trauma, etc. and coordinating services with other agencies.

  • Identify and leverage clients’ strengths as part of service planning to help clients achieve their self-defined goals.
  • Work with teammates and other support staff to ensure fidelity of the Strengths Model of Case Management is adhered to and considered in interaction with and documentation of client-case manager engagement.
  • Assess clients mental and physical wellness, needs, preferences, and abilities and utilize these to develop tailored wrap-around support.
  • Support and guide clients from homelessness to housing by helping navigate potential housing options, landlords, barriers, and obtaining documentation needed in order to secure permanent housing.
  • Develop and maintain positive working relationships with community contacts, such as referral sources, landlords, and other service providers.
  • Collaborate with various community programs and agencies to advocate for client population needs and provide appropriate referrals;
    Keep apprised of available community resources.
  • Regularly and effectively assess and manage crisis situations involving both psychological and environmental components; decide on appropriate response in crisis situations and act in accordance with standards and laws as indicated.
  • Adapt quickly to critical needs of clients and landlords that might involve mutual recission, VAWA, and other high risk situations.
  • Manage multiple requirements of a variety of funders in terms of data, paperwork, processes and procedures.
  • Meet with individual clients to establish a safe, healthy relationship for learning, problem-solving, and participation in case management and community activities.
  • Maintain case records and documentation as appropriate, including service plans, assessments, progress notes, intake reports, and incident reports.
  • Complete interim and annual assessments in a timely manner in order to meet expectations of contract and homeless response system requirements.
  • Set up utility accounts in client’s name, review previous rental and utility arrears, and negotiate late fees on behalf of clients.
  • Analyze barriers and challenges as they arise and deliver safe, effective, and youth-focused solutions that are in accordance with Life Works policy and philosophy.
  • Support and partner with youth and actively engage in the community alongside clients as necessary and indicated by client’s self-defined goals.
  • Communicate with members of the community, colleagues, and clients effectively and efficiently.
  • Prepare for and actively participate in supervision meetings and all other required meetings or trainings.
  • Transport clients to agencies and services, as needed.
