Registered Nurse

Overview

Registered Nurse role at Sevita – Mentor Community Services. Salary $32.00/hr (Full‑time and PRN).

7:00 p – 7:00 a.

Location:

College Drive, Avon Park FL 33825.

• Provide nursing services and support for individuals with developmental disabilities, acquired brain injuries, substance abuse issues, and emotional/behavioral challenges.
• Conduct medical assessments and determine placement in the community.
• Develop and implement service plans and supervise medical components in collaboration with physicians.
• Review and revise nursing care plans regularly.
• Maintain accurate medical records, medication administration records, and physician orders.
• Coordinate with primary care physicians and specialists; attend doctor visits as directed.
• Promote health and wellness, prevent complications, and administer medications per guidelines.
• Oversee management of high‑risk medical conditions and private duty nursing services.
• Provide medical skills training to clients, mentors, respite providers, staff, and family.
• Participate as a team member in Individual Health Plans (IHP) and Plans of Care (POC).
• Assist with home‑care issues, durable medical equipment, occasional light housekeeping and meal preparation, transport, and on‑call duties.
• Attend IDT meetings and relevant committees as needed.

• Administer medications and document compliance.
• Deliver consultation and technical assistance regarding home‑care issues.
• Coordinate household assistance, transport, and facilities.
• Participate in on‑call system as required.
• Perform other related duties as requested.

• Graduate of an accredited RN program.
• Minimum one year experience caring for medically complex individuals.
• Current state RN license.
• Current driver’s license, vehicle registration, and auto insurance for transportation duties.
• Current CPR/First Aid certification.

• Lifting up to 100 pounds occasionally, 50 pounds frequently, and 20 pounds continuously.

• Knowledge of nursing care plans, medication administration, IHP/POC development, and home‑care equipment.
