Patient Services Coordinator - Integrative Medicine (Per Diem) at Hartford Health Care

Shift: M-F 8a-4:30pm Per Diem

- Gathers pertinent information from insurance carriers, financial counselors, and other ancillary staff to make certain the patient’s financial obligations for services provided
- Processes paperwork for new patients efficiently and in accordance with protocols. Verifies insurance coverage and obtains authorizations if needed from insurance carrier. Resolves pre-certification, registration and case-related concerns prior to a patient’s appointment
- Tracks and supports patients when they obtain services outside the practice e.g. Emergency departments, Hospital and other health care facilities.
- Monitors and meets PCMH referral processes.
- Enteres referrals and documents communications, actions and other data in an information system. Ensures that all referral requests are properly signed and filed.
- Care coordination – assists with specialty appointments and obtain needed tests recommended by specialist.
- Communicates with other team members and referral teams constructively in order to reduce conflicts and enhance the resolution of issues.
- Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns on orders and referrals.
- Working collaboratively with other team members, provide care management services for high risk patients.
- Obtains insurance referrals for patients, getting PA for diagnostic imaging and sending referrals to specialist office for cardiac and preventative screenings.
- Associate’s degree or equivalent knowledge in ambulatory environment.
- Five years experience working with referral processes.
- Familiarity with EPC EHR system
