Job Summary

As the Nurse Designee, you will manage the successful operation of the Resident Care Department under the leadership of the Resident Care Director (RCD). You are responsible for the oversight, coaching, and direct supervision of all resident care service providers in the community, including resident assessment and management as defined by the RCD. In the absence of the RCD, you will fill in as the leader of the Resident Care Department.

Principal

Duties / Responsibilities
  • Demonstrate expertise in clinical skills and nursing judgement within your scope of practice.
  • Complete a thorough assessment of potential residents; obtain the medical information release form from the primary care physician as needed.
  • Develop a plan of care (in collaboration with the RCD) based on identified physical and psychological problems, resident-specific goals, and planned program interventions.
  • Create a service plan (in collaboration with the RCD) that meets state regulatory requirements; accurately assess, communicate, and document residents’ status.
  • Coordinate care needs with community providers via an effective case‑management process.
  • Measure and record vital signs (temperature, pulse, respiration, blood pressure, weight, and height) where appropriate per state regulations.
  • Observe and report significant changes in resident behavior and health to the RCD, physician, family, RCAs, and Executive Director per state regulations.
  • Recognize and assist in the prevention of elder abuse, neglect, and exploitation and report to appropriate sources per state regulations.
  • Direct and supervise daily assignments of Resident Care Assistants; serve as first point of contact for assignment questions or concerns.
  • Participate in hiring and termination processes for resident care associates, sourcing and screening new candidates and facilitating terminations.
  • Provide input into performance reviews, merit increases, and necessary disciplinary documentation for care associates; present reviews with the RCD.
  • Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their job.
  • Ensure compliance with all resident‑care related regulations (e.g., medication management).
  • Maintain infection prevention and control; personal and resident safety.
  • Ensure physician and family notification per state regulations.
  • Participate in the QA process per assignment.
  • Provide ongoing supervision of residents’ health‑related activities, promptly seeking medical attention when needed.
  • Conduct documentation audits as required.
  • Function as Resident Care Director as required.
  • Participate in training/in‑service as required by Benchmark and state regulations.
Education / Experience / Licensure / Certification
  • Graduate of an approved RN program (per state requirements).
  • Minimum 1–2 years of experience working as a nurse in a Home Health, SNF, and/or Assisted Living facility.
  • Previous experience working with the elderly in a paid or volunteer position highly desirable.
  • Valid, unrestricted RN state license.
  • Current CPR, First‑Aid, and Heimlich certification, if required.

  • Maintain confidentiality regarding residents, staff, and the Community.
  • Proficient with MS Office and electronic health records.
