We’re seeking an experienced Nurse Practitioner or Physician Assistant to join our adult primary care team in Avon, CT, where a tight‑knit and highly collaborative culture sets the tone for a positive work environment. Role: Open to Nurse Practitioners (APRN) and Physician Assistants (PA)
Location: Avon, CT
Setting: 100% Outpatient Primary Care – Adult Medicine (18+)
Schedule: Full‑time, weekdays only!
Call schedule: No call required
Support for Nurse Practitioners and Physician Assistants- Dedicated support and voice for our NPs and PAs through the Office of Advanced Practice directed by Sarah Clark, MBA, MHS, PA‑C, BSN, RN, System Director of Advanced Practice

Under established guidelines and in collaboration and/or supervision with the practice physician(s), NPs and PAs perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients. In consultation with the physician(s) and care team members, NPs and PAs develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists. NPs or PAs provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identify problems outside of their scope of practice and refer appropriately to physician.
Qualifications- Licensure:
Unrestricted State of Connecticut Medical License (or eligible) - Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
- Experience:
2+ years’ experience in a primary care or relevant outpatient setting
