Dialysis Clinic, Inc. is seeking a Home Therapies Nurse to support our patient-centered care model across home hemodialysis and peritoneal dialysis programs. The role combines clinical duties with patient education and coordination of training sessions, clinic visits, and home visits.
You will collaborate with a multidisciplinary team to ensure safe, effective home treatments, monitor patient parameters, and maintain thorough documentation in line with ESRD network and clinic policies.
