This role offers an opportunity to provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes while working closely with a dedicated team of physicians and clinicians.

$70,500.00/yr – $96,900.00/yr

• Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, with follow‑up as directed by the Clinical Manager.
• Confer with physician in developing the initial plan of treatment based on orders and patient assessment; revise plan in consultation with physician as needed.
• Coordinate care with physical therapists, occupational therapists, home health aides, and external providers.
• Report patient care and progress to physician and Clinical Manager continuously.
• Implement patient care plan in collaboration with patient and family to achieve optimal outcomes.
• Coordinate/oversee/supervise the work of home health aides, providing written care instructions and monitoring completion of documentation.
• Discharge patients after consultation with the physician and Clinical Manager, completing required clinical documentation.
• Prepare comprehensive medical documentation, including case conferences, medication changes, re‑certifications, and care plan updates.

• Diploma, Associate or Bachelor degree in Nursing.
• Minimum of one year of nursing experience preferred.
• Strong acute care experience (med‑surg, ICU, ER, acute setting).
• Home health experience a plus.
• Current and unrestricted Registered Nurse license.
• Current CPR certification.
• Strong organizational and communication skills.
• Valid driver’s license, auto insurance and reliable transportation.

Scheduled hours: 40 hours per week.
