As a Social Care Assistant working within the Home Independence Team, you will be working out in the community to support residents who have recently been discharged from hospital or who have a disability and need help living independently. You will support individuals to regain confidence and achieve their goals, focusing on personal wellbeing outcomes and individual wishes. The team works closely with healthcare professionals, carers, clients and families to develop suitable support packages and enhance client independence.

You will enjoy working in a fast‑paced environment with the wellbeing of our residents at heart.

Responsibilities
  • Provide information and advice in relation to care and support and assist individuals in accessing care through various means.
  • Deliver care as part of a CQC‑registered service in the community, maintaining autonomy and using initiative to comply with regulations.
  • Ensure agreed care plans meet individuals' outcomes and empower them to access early intervention and preventive services.
  • Inform senior colleagues and adhere to safeguarding procedures for any concerns about vulnerable adults.
  • Partner with individuals, carers, service providers, voluntary sector and other professionals.
  • Provide personal care, including dressing, toileting, washing and general housekeeping.
  • Demonstrate empathy, care, problem‑solving and use remote management oversight as required.
  • Utilise technology‑enabled care devices and occupational therapy aids to support independence.
Eligibility & Requirements

This position is community based, not day‑centre based. A full UK driving licence and access to a vehicle are essential.

  • Full UK driving licence and vehicle access.
  • Car user allowance and a 45p per mile mileage claim allowance (HMRC mileage allowance).
  • Exempt from the Rehabilitation of Offenders Act 1974; applicants must declare:
  • – All unspent convictions and conditional cautions.
  • – All spent convictions and adult cautions that are not protected.
