If so, this role may be for you.
The Urgent Community Response Frailty Hospital at Home (UCR FH@H) team delivers hospital-level urgent and same-day responses to people in their usual place of residence following a sudden deterioration in their health or condition. This could be a one-off visit or a short episode of care, delivered through multidisciplinary team working and working collaboratively with other community services.
