Overview
The post holder will deliver an expert healthcare service to adults within HMP Aylesbury. They will offer advice to clinicians and allied healthcare professionals, providing a comprehensive primary care physical health service within an integrated healthcare system that includes primary care, wellbeing, mental health and substance misuse. The role involves conducting physical health assessments, running clinics, delivering health treatments and interventions, and contributing to continuous service development.
The post holder will demonstrate knowledge and skills to provide clinical guidance to staff, promote high standards of care, safeguard vulnerable patients, manage risks, make referrals, coordinate multidisciplinary practice and work within their competency, ensuring training needs are communicated and quality of care is enhanced through audits and research.
- Plan, deliver and evaluate interventions for treatment using evidence‑based models.
- Set up protocols for effective assessment, planning, implementation and evaluation of care for patients with physical, mental health and substance misuse problems.
- Safeguard vulnerable patients in line with Trust safeguarding policy.
- Apply local risk assessment and management protocols.
- Make appropriate referrals to other services for longer‑term interventions.
- Coordinate multi‑disciplinary clinical practice to provide focused and integrated care programmes.
- Manage aggressive patients and follow appropriate procedures.
- Work collaboratively with staff from other disciplines, including officers and governors, to provide integrated care.
- Operate within own competency and communicate training needs to the team manager.
- Participate in local and Trust audits, clinical governance and research for service development.
- Maintain efficient pathways of care, collect collateral information, and ensure timely community referrals at discharge.
- Communicate risk information sensitively and comprehensively to colleagues.
- Keep accurate clinical record keeping, including electronic data entry, and consider diversity issues in patient care.
- Maintain standards of professional communication with patients, relatives and colleagues.
– Essential Criteria
- Registered General Nurse.
- Evidence of personal and professional development in Nursing since registration.
- Willingness to undertake further training relevant to the clinical area.
- Knowledge of medication rounds and safe storage and administration of drugs.
- Knowledge of and ability to follow the NMC code of conduct.
- Ability to manage work under pressure and demonstrate stress, conflict management and problem‑solving skills.
- Ability to assess plans, implement and evaluate patient care and act accordingly if untoward changes occur.
- Knowledge of risk assessment and management and its implementation.
- Ability to treat patients with respect and dignity.
- Computer skills (access patient information, email, MS Word, Excel).
- Ability to maintain patient records within local Trust and NMC standards.
- Understanding of multi‑disciplinary team roles and own role within it.
- Evidence‑based practice, understanding of clinical governance and quality of care.
- Experience of providing high‑quality care and exceeding expectations.
- Leadership skills (motivating, initiative, change management).
- Post‑registration qualification appropriate to speciality.
- ENB 998 or equivalent.
- Chronic disease management or long‑term condition qualification.
- Experience of working within a secure environment.
- Experience of system IT system.
- Experience at senior band 5 grade.
- Experience in providing and receiving supervision.
- Experience running clinics and/or group work.
- Ability to keep accurate records.
- Practical experience of providing high‑quality care and exceeding expectations.
