Adult’s Social Worker, Learning Disability / Transitions Team, Aylesbury

Pay rate £37 per hour. Contract role, LD team.

Pertemps are recruiting for an experienced Adult’s Social Worker in Buckinghamshire.

This is to work in a Learning Disability / Transitions Team. The Transitions Team work with young people, their families, education, health and further networks to support the transition into Adult Social Care. This involves Care Act Assessments, just prior to 18th birthdays.
