About the Role:

An exciting opportunity has arisen for an experienced Qualified Social Worker to join a Children with Disabilities Team in Aylesbury, Buckinghamshire
.

The successful candidate will work alongside children, young people and their families to assess needs, coordinate appropriate support and promote positive outcomes. You will play a key role in ensuring children with disabilities receive high-quality, child-centred social work support while safeguarding their wellbeing and promoting independence.

This is an excellent opportunity for a dedicated practitioner with experience in children's social care who is passionate about improving outcomes for children and families.

Key Responsibilities:
  • Manage a caseload of children and young people with disabilities and complex needs.
  • Undertake comprehensive assessments and reviews in line with statutory requirements.
  • Develop and implement effective, child-centred care and support plans.
  • Work closely with children, young people, parents, carers and wider family networks.
  • Identify and respond appropriately to safeguarding concerns and risks.
  • Coordinate services and support packages to meet assessed needs.
  • Work collaboratively with education, health, care providers and other partner agencies.
  • Ensure children and families are actively involved in planning and decision-making.
  • Maintain accurate, timely and high-quality case records and reports.
  • Ensure statutory visits, assessments and reviews are completed within required timescales.
  • Promote strengths-based practice and positive outcomes for children and young people.
