Paramedic Practitioner

Job Summary

You will be a degree qualified Paramedic Practitioner with relevant clinical skills and expertise to work autonomously. You must have relevant experience enabling you to independently assess, diagnose, treat and refer patients as appropriate, as well as manage your own patient lists. Clinicians are available for guidance if required.

The paramedic practitioner will work autonomously in the practice, providing a range of services such as assessment, diagnosis, treatment, telephone triage and referring patients appropriately, as well as supporting the management team in reviewing clinical policy and procedure.

Aylesford Medical Centre is a GMS Practice serving approximately 6,850 patients.

We occupy a purpose built building in the Royal British Legion Village, close to junction 5 of the M20 and have our own car park.

We use the EMIS Web clinical system.

• Identify, signpost or refer patients at risk of developing long-term conditions, preventing adverse effects on the patient’s health
• Provide routine care to patients as required in accordance with clinically based evidence, NICE and the NSF
• Examine, assess and diagnose patients and provide clinical care/management as required
• Triage patients and provide the necessary treatment during home visits
• Triage and treat or refer patients who attend the surgery or over telephone
• Collect pathology specimens as required
• Maintain accurate clinical records in line with extant legislation
• Ensure that Read codes are used effectively
• Ensure continuity of care, arranging follow-up consultations or reviews as necessary
• Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes
• Chaperone patients where necessary
• Prioritise health issues and intervene appropriately
• Act as the lead clinician when dealing with clinical emergencies
• Recognise, assess and refer patients presenting with mental health needs
• Support patients in the use of prescribed medicines or over-the-counter medicines (within own scope of practice)
• Liaise with external services/agencies to ensure that the patient is supported appropriately (vulnerable patients, etc.)
• Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times
• Support the clinical team with all safeguarding matters, in accordance with local and national policies
• Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
• Deliver opportunistic health promotion where appropriate

• Registered Paramedic (HCPC)

• Experience of working within a primary care setting
• Experience of working as a paramedic practitioner
