An exciting opportunity has arisen for a registered staff nurse to join the Hospital at Home (H@H) service currently based within Ailsa Campus.

From 1 April 2026, the working week for NHS Agenda for Change staff in Scotland will be reduced. Full-time hours will reduce from 37 to 36 hours (pro rata for part-time staff) without loss of earnings. Shift pattern is Monday – Sunday, 08.00-20:30, 365 days of the year.

The role of staff nurse within H@H will utilise high levels of decision making and clinical judgment within a new and growing service. It is essential that patient health needs are met through the provision of person led care and management of patient conditions through the assessment, treatment and review planning process in partnership with colleagues, patients and families. A high level of effective communication and collaboration is required on a daily basis.

As a nurse you will be working in line with the professional standards held in the NMC Code. You will work as part of the team to lead and develop the service to improve practice and work collaboratively across healthcare boundaries, implementing and evaluating evidence based standards, guidelines and policies. The staff nurse within the H@H team has a key role in the person led care and treatment of acute and complex patients within the community setting.

H@H provides multi-disciplinary assessment and treatment of patients with both acute and long term conditions. NHS Ayrshire & Arran has a population of approximately 376,000 patients across – Acute Services & the Health and Social Care Partnerships in East, North & South Ayrshire. The post holder will visit patients who are in their own homes, community hospitals, care homes and step-down units.

The post holder will have the support of a senior clinician to enhance the ongoing development of the team and staff members. The post holder is employed within NHS A&A and there may be a requirement to work flexibly across Ayrshire & Arran to meet service demands.

Knowledge and experience of providing direct clinical care that encompasses assessment, treatment and management of frail patients who are acutely unwell and make timely decisions regarding their management. This involves making onward appropriate consultant / clinic / rehabilitation / MDT referrals and to discharge patients directly from the service.

Treat and manage a large and delegated clinical caseload under supervision and to establish and develop skilled practice in the multidisciplinary management and care of frail patients with a range of acute conditions within homely settings.

Have the ability to assess and manage significant clinical risk within own delegated caseload – this involves quick and effective identification of serious conditions/deterioration and the necessary escalation to Advanced Nurse Practitioners and other healthcare professionals, which may…
