Frontpoint Health is a fast‑growing home health agency committed to delivering personalized, high‑quality care. We are seeking experienced and compassionate Physical Therapists to help patients recover safely and comfortably in their homes.

  Azle, TX
• Schedule:
  
  Day shift, Monday to Friday, optional weekend availability;
  Full‑time or Part‑time available.

• Provide in‑home physical therapy to patients recovering from injury, surgery, or illness.
• Evaluate patients’ physical conditions and develop individualized care plans.
• Implement therapeutic exercises and mobility techniques.
• Educate patients and caregivers on rehabilitation best practices.
• Collaborate with nurses, physicians, and care teams.
• Complete timely and accurate documentation in our EMR (Kinnser/Well Sky).

• North Dallas
• South Dallas
• Duncanville, DeSoto, Lancaster, Glenn Heights, Cedar Hill, Seagoville, Balch Springs, Hutchins, and other South Dallas locations.

• Active Texas Physical Therapist (PT) license (Required).
• 1+ year of home health experience (Required).
• Reliable transportation and valid driver’s license (Required).
• Familiarity with Kinnser/Well Sky (Preferred).
• Excellent time‑management and communication skills.
• CPR certification (Preferred).
