As an Independent Optometrist leasing space within Walmart, you will take a leading role in patient care, including eye and vision services as well as general health and well-being.

• Launch and manage your services in Walmart stores.
• Provide patient care for eye and vision services.
• Use your autonomy to establish days of practice, hours of operation, patient schedule, and fee schedule.
• Keep 100% of exam/professional fees.

• Fair market value equipment/instrumentation.
• Administrative and/or Opt-Tech support services (if state allows).

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 form.
