Community Health Worker - Bakersfield

Through early interventions designed around social determinants of health (SDoH), world-class care focused on chronic disease, and human-centered technology, we believe we can build a healthcare model that allows for all patients to live their fullest lives.

We are a fast-growing, impact-focused, YC-backed company created out of the University of Southern Californias D-Health Lab with the idea that empowering government, healthcare, and nonprofit services with population-based data could help usher in a new era of preventive public health. Today we are building a full stack medical network that leverages our predictive capabilities to provide a frictionless experience for both our patients and care providers.

• Manage a caseload of patients. This includes completing assessment forms with patients, developing care plans (with patients and clinical staff), and carrying out activities according to the care plan.
• Build rapport with patients with a goal of increasing the likelihood of positive behavior changes.
• Coach patients to minimize risks associated with the identified common health conditions and behaviors.
• Accompany members to medical and social services appointments.
• Connect members to appropriate programs to address barriers to care.
• Link members to local, county and state services. Follow up with members and serve as a member advocate.
• Document information from every encounter in designated information systems.
• Outreach and engage with eligible patients to enroll them into the Akido ECM Program.
• Other duties as assigned.

• High School Diploma or equivalent
• Bilingual in English and Spanish strongly preferred
• 2+ years of experience with and comfort working with the following populations: people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration
• Ability to work in a dynamic, outdoors environment
• Ability to work independently as well as part of a team
• Ability to prioritize multiple and competing tasks
• Ability to communicate effectively, including articulating ones own relevant personal experiences
• Maintenance of reliable transportation
• Proficiency in operating a computer and documenting information into designated systems
• Travel 50-75% — Must have a valid drivers license, automobile insurance and reliable transportation

$25/hr

Salary range

$43,680 - $52,000 USD
