Compassionate Care Home Health Agency has five office locations in Fresno, Visalia, Modesto, Merced, and Bakersfield and serves eight Central Valley counties. We maintain a culture of respect and love for patients and team members, with a commitment to quality care and employee growth.

Hourly rate range: $33–36 (negotiable based on experience). Salary option available. Opportunity to accumulate overtime.

• Provides direct patient care as defined in the State Nurse Practice Act, including end‑of‑life care.
• Implements current nursing practice following a comprehensive assessment and plan of care.
• Provides accurate and timely documentation of patient services.
• Assesses and provides patient and family/caregiver education and information relevant to diagnosis and plan of care.
• Participates in coordination of hospice services and reports identified needs to the interdisciplinary group.
• Uses and prepares equipment and supplies effectively, adhering to aseptic technique.
• Provides appropriate pain/symptom management and evaluates patient response to treatments/medications.
• Participates in personal and professional growth and development and quality improvement activities.
• Performs other duties as assigned by the registered nurse.

• Graduate of an accredited practical nurse or vocational nursing program.
• One year of nursing experience; three years preferred.
• Sensitivity to patient needs with compassion in dealing with them.
• Currently licensed as an LPN or LVN in California.
• Good verbal and written communication and organization skills.
• Licensed driver with an insured, safe automobile.
• Strong commitment to providing the highest quality care.
• Current CPR certification.
• Understanding of hospice philosophy and terminally ill needs.
• Excellent communication and listening skills; positive, “can‑do” attitude.
• Confidence and creative problem‑solving skills.
• Flexibility and availability for schedules outside standard hours, including possible weekends.
