Kona Medical Consulting and The Home Health Team is actively looking for a Home Health Field Registered Nurse (RN) in Dallas-Fort Worth Metroplex.

We are seeking a skilled and motivated Home Health Field Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. The ideal candidate is reliable organized confident in clinical decision-making and passionate about helping patients achieve their health goals.

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care disease management medication administration and patient education
• Communicate effectively with physicians case managers and interdisciplinary teams
• Complete accurate timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families

• Current RN license in the state of Texas
• 1 year of nursing experience (Home Health preferred but not required)
• Strong clinical assessment and critical-thinking skills
• Reliable transportation valid drivers license and proof of auto insurance
• Comfortable using EMR / mobile charting
