Your expertise will ensure safe and effective transitions post-hospitalization, making a real impact on patient outcomes.
YourRole and Responsibilities:
In this dynamic position, you will:
- Assess, plan, advocate, and coordinate care throughout the patient’s journey.
- Collaborate with an interdisciplinary team to optimize patient outcomes.
- Utilize performance metrics and best practices to enhance quality and safety.
- Focus on effective transition planning and length of stay oversight.
To thrive in this role, you should meet the following qualifications:
- Education:
- Graduate of an accredited School of Nursing (required).
- Bachelor's Degree in Science in Nursing (minimum required).
- Experience:
- 2-3 years in an acute care hospital setting (preferred).
- Experience in Care Management or Utilization Management (preferred).
- Licensure: Current License in the state of employment (required).
- Certifications:
- BLS (CPR) at hire date (required) or within 90 days of hire.
- Certification in Case Management (preferred).
