Are you ready to make a difference in patient care? As an RN Care Manager II-IPCM, you will play a vital role in coordinating and managing patient care from admission to discharge. Your expertise will ensure safe and effective transitions post-hospitalization, making a real impact on patient outcomes.

In this dynamic position, you will:

• Assess, plan, advocate, and coordinate care throughout the patient’s journey.
• Collaborate with an interdisciplinary team to optimize patient outcomes.
• Utilize performance metrics and best practices to enhance quality and safety.
• Focus on effective transition planning and length of stay oversight.

To thrive in this role, you should meet the following qualifications:

• Education:
  • Graduate of an accredited School of Nursing (required).
  • Bachelor's Degree in Science in Nursing (minimum required).
• Experience:
  • 2-3 years in an acute care hospital setting (preferred).
  • Experience in Care Management or Utilization Management (preferred).
• Licensure: Current License in the state of employment (required).
• Certifications:
  • BLS (CPR) at hire date (required) or within 90 days of hire.
  • Certification in Case Management (preferred).
