Are you ready to make a difference in patient care? As an RN Care Manager, you will play a vital role in coordinating and managing patient care transitions from admission to discharge. Your expertise will ensure safety and quality throughout the hospitalization process and beyond.
Your Role:As part of our interdisciplinary team, you will:
- Assess, plan, advocate, and coordinate care for patients.
- Ensure safe transitions post-hospitalization.
- Utilize best practices and performance metrics to optimize length of stay and enhance patient experience.
- Work in alignment with Mercy's mission, values, and service standards.
Required
Education:
- Graduate of an accredited School of Nursing
- Bachelor's Degree in Science in Nursing
- 2-3 years in an acute care hospital setting (preferred)
- Experience in Care Management or Utilization Management (preferred)
- Current License in the state of employment (required)
- BLS (CPR) certification at hire date (required) or within 90 days of hire
- Certification in Case Management (preferred)
