Are you ready to make a difference in patient care? As an RN Care Manager II-IPCM, you will play a vital role in coordinating and managing patient care from admission to discharge. Your expertise will ensure safe transitions and quality outcomes, all while supporting the Mercy mission through best practices and performance metrics.
Your Role and ResponsibilitiesAs a key member of our interdisciplinary team, you will:
- Assess, plan, advocate, and coordinate care throughout the patient’s journey.
- Ensure safe transitions post-hospitalization.
- Utilize performance metrics and best practices to enhance patient experience and safety.
- Maintain oversight of length of stay to optimize care quality.
Required
Education:
- Graduate of an accredited School of Nursing.
- Bachelor's Degree in Science in Nursing (minimum required).
Experience:
- 2-3 years in an acute care hospital setting (preferred).
- Experience in Care Management or Utilization Management (preferred).
Licensure:
- Current nursing license in the state of employment (required).
Certifications:
- BLS (CPR) certification at hire date or within 90 days (required).
- Certification in Case Management (preferred).
