The Department of Pharmacy prioritizes teaching, research, and patient care. It provides acute care pharmacy services for a diverse population of medical and surgical patients, with additional services in the Johns Hopkins Outpatient Center and affiliated clinics, and includes 45 acute care and primary care clinical pharmacy specialists and 31 general and specialized pharmacy residents.

The Johns Hopkins Hospital seeks a motivated individual to assume an Anticoagulation Clinical Pharmacy Specialist role. The specialist will collaborate with physicians, advanced practice providers, nurses, and other pharmacists to provide comprehensive medication management. Responsibilities include provision and oversight of anticoagulation pharmacotherapy for patients in an ambulatory anticoagulation clinic (located at Johns Hopkins Bayview Medical Center) and in the Center for Perioperative Optimization (located at The Johns Hopkins Hospital).

The specialist will also support the hemostatic and antithrombotic stewardship program and transitions of care for patients.

The specialist will develop therapeutic plans, monitor outcomes, and provide patient education, with opportunity for advanced practice via a drug therapy management agreement to initiate, modify, continue, and discontinue medication therapy.

Additional duties include providing education to students, residents, pharmacists, and other healthcare practitioners, serving as a preceptor for pharmacy residents in ASHP‑accredited PGY‑1 and PGY‑2 specialty programs, supporting and leading quality improvement and research projects, and participating as an active member of hospital and departmental committees. Faculty appointments are available through affiliations with the University of Maryland College of Pharmacy, the University of Notre Dame of Maryland College of Pharmacy, and the Johns Hopkins University School of Medicine.

Preferred qualifications:

completion of a Pharm

D degree, PGY‑1 residency or equivalent experience, and PGY‑2 specialty residency or equivalent experience in thrombosis and hemostasis management, ambulatory care, internal medicine, or pharmacotherapy. Board of Pharmacy Specialties (BPS) certification is required within two years of start date.

Salary Range: Minimum $58.41/hour – Maximum $96.46/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
