Licensed Social Work Care Coordinator will provide short‑term care coordination services to individuals and families utilizing MD Medicaid insurance. Responsibilities include receiving referrals, assessing client needs, partnering with Medicaid Managed Care Organizations (MCOs), providers, public health and community resources to address barriers. The coordinator will lead a team to resolve complex psychosocial and behavioral health conditions.
Responsibilities- Participate in professional development opportunities in client engagement, motivational interviewing, trauma‑informed outreach strategies.
- Attend local/state meetings, conferences, community meetings and city‑wide initiatives.
- Consistent, timely communication with supervisor – SW Program Manager.
- Host team meetings with outreach staff to discuss complex cases, share best practices and resources.
- Initiate client outreach via telephone or home visit within 5‑10 days of referral receipt.
- Outreach clients with medical or psychosocially complex needs; assess and educate on Medicaid benefits; reconnect recipients to medical home and link to covered benefits.
- Consult with community agencies to identify resources for resolving social determinants of health.
- Connect & collaborate with providers, hospitals, community organizations, MCO case managers and other professionals.
- Assess, triage, and refer clients to available resources; maintain a monthly minimum success rate of 70% and on‑time case closure rate of 90%.
- Document all outreach activities, client assessments, activity, service plans and resolutions; prepare narrative and statistical reports.
- Complete and submit timesheets and required training courses.
- Bachelor’s degree in social work from an accredited college or university.
- Current licensure as a Social Worker in the State of Maryland.
- Minimum of 2 years of related experience.
- Relevant clinical experience providing services to pregnant women and/or infants, preferably in a public health/community setting.
- Knowledge of community outreach, Medicaid system navigation, Health Choice Medicaid recipients and MCOs.
- Strong organizational, problem‑solving and time‑management skills.
- Excellent oral and written interpersonal and communication skills.
- Proficient with MS Word, Excel, Outlook and SharePoint (365).
