PRIMARY DUTIES:- Provide advanced clinical services including performing athorough medication reconciliation and assessment to identify,prevent, and resolve medication-related problems.
- Provide advanced clinical services for individuals with acuteand chronic pulmonary and sleep-related breathing conditions,including, but not limited to, asthma, chronic obstructive pulmonary disease (COPD), bronchiectasis, interstitial lung disease(ILD), pulmonary hypertension, pulmonary arterial hypertension(PAH), obstructive and central sleep apnea, and other complex cardiopulmonary disorders.
- Perform comprehensive medication reconciliation and assessment to identify, prevent, and resolve medication-related problems.
Recommendations are documented during the clinic encounter and communicated to the clinic provider or, when appropriate, to the patient's outside healthcare providers. Follow-up appointments are scheduled as clinically necessary. - Manage discharge medication needs and provide medication education for patients with COPD who are admitted to BWMC and subsequently discharged for post-hospital follow-up in the pulmonary clinic.
- Provide transitional care pharmacy services to support patients transitioning from the hospital to the outpatient clinic, including participation in rounds, clinic visits, and interdisciplinary treatment team meetings.
- Evaluate the clinical and financial impact of specialty medications and high-cost pulmonary therapies; identify opportunities to optimize site of care, medication acquisitionstrategies, formulary selection, and benefit utilization.
- Participate in quality improvement initiatives and clinical pathway development to enhance service quality, productivity, and institutional best practices.
- Provide institutional service through committee participation and other activities that support the hospital and pharmacy program.
- Precept PharmD students and pharmacy residents and contribute approximately 10% effort to the School of Pharmacy Pharm Dcurriculum including on-site assignments.
