Clinical Nurse (DOM-Infectious Diseases)

We are seeking a Clinical Nurse that will support the Baltimore City Health Department's (BCHD) Mobile Clinical Services program that delivers specialized health care services to people who use drugs (PWUD) and individuals at risk or living with HIV, hepatitis C (HCV), sexually transmitted infections (STIs), opioid use disorder (OUD), and chronic or acute wounds. The Clinical Nurse will be an integral part of a multidisciplinary care team that includes clinicians, case managers, and community health workers.

The Clinical Nurse will coordinate care across the full continuum, perform direct patient care within the scope of nursing practice, and serve as a resource and advocate for patients navigating healthcare and social services systems. This position is primarily based on The Spot Mobile Clinic Vehicles, operating four days per week in high-need neighborhoods, and will also include telephonic engagement through BCHD's buprenorphine outreach program.

Specific

Job Responsibilities Patient Care & Case Management
  • Provide patient education and program information regarding HIV, HCV, STI, OUD, wound care, and buprenorphine services at mobile and telemedicine sites.
  • Perform intake assessments and nursing evaluations with a focus on social determinants of health, STIs, opioid use, and harm‑reduction; document accurately in the EMR, order laboratory testing, and provide harm‑reduction resources based on these assessments.
  • Collect and process laboratory specimens (or delegate to support staff), ensuring adherence to established laboratory protocols and procedures.
  • Develop individualized care plans in collaboration with the Mobile Clinical Services team.
  • Administer nursing care consistent with Maryland RN scope of practice, including assessments, screenings, test ordering, medication, and immunization administration under standing orders.
  • Perform follow‑up visits (in person and by phone) to support continuity of care, medication education, and adherence.
  • Provide crisis intervention and triage for complex cases.
  • Coordinate patient transitions to primary care, behavioral health, specialty services, and community resources.
  • Advocate for patients with insurers, pharmacies, and other service providers to ensure continuity of care.
  • Support quality improvement initiatives and identify opportunities to address unmet patient needs.
  • Assist with maintaining clinical and lab supply inventory.
Clinical Nursing Functions
  • Assist with minor procedures, wound care, specimen preparation, injections, and other clinic‑based treatments per Maryland Board of Nursing guidelines.
  • Serve as a resource to nursing staff, clinical support staff, and other health care providers.
  • Provide staff training, competencies, and serve as a preceptor / mentor when needed.
  • Communicate with patients regarding test results, prescriptions, and physician instructions.
  • Participate in patient and staff education, counseling, and health promotion activities.
Minimum Qualifications
  • Individual must be a registered nurse, licensed in the State of Maryland.
  • Additional education may substitute for required experience, and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula.
Preferred Qualifications
  • Bachelor's Degree in Nursing or related discipline.
  • Prior experience in wound care (WOCN certification desirable).
  • Clinical experience in HIV, HCV, STI, and/or substance use care.
