As a Care Manager at Elder Tree, you will play a crucial role in ensuring that our clients receive the highest quality of support. This role blends clinical judgment with relationship‑driven care, supporting clients and families through complex medical, social, and life transitions.

Role Description

The Nurse (RN/LPN) Care Manager is a critical part of the Care team, serving as a medical advocate for clients and guiding our multi‑disciplinary team through various clinical situations. The role requires the ability to work independently across various clinical settings including homes, hospitals, SNFs, and senior living facilities in the metro DC area.

Responsibilities include:

• Comprehensive Care Management:
  Oversee and coordinate tailored care plans, ensuring that our clients receive appropriate and timely services.
• Client Advocacy:
  Serve as a strong advocate for clients, ensuring their needs are met in home, medical and care facility settings, including while communicating with healthcare providers.

  Collaboration:
  
  Work closely with team members to address clients’ needs, and contribute your expertise to the team, helping to enhance the collective knowledge and skills within Elder Tree.
• On‑Call Availability:
  Participate in after‑hours or weekend on‑call rotations as needed to provide continuous support to clients.
• Field Work and Documentation:
  Update family members and other members of the care team, and maintain thorough and accurate documentation. Approximately 90% of the role involves field work, with 10% dedicated to follow‑up and office‑based tasks.

Qualifications

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in VA, MD, or DC
• Strong communication and interpersonal skills for engaging with clients and families
• Ability to collaborate with healthcare providers and local care resources effectively
• Strong problem‑solving, organizational, and time‑management abilities
• Proficiency in maintaining accurate client records and documentation
• Compassionate approach with a focus on client autonomy and well‑being
• Experience in elder care, geriatrics, or a related healthcare field is a plus
• Available 9am to 6pm Monday‑Friday and some weekends
• Must be able to lift and maneuver up to 50 pounds
• Must have a car with a good driving record and be comfortable and willing to drive regularly around the DC Metro area (tolls and mileage reimbursed)
