Description:
RN/Patient Care Coordinator for pediatric patients.
This role involves supporting the Pediatric Complex Care Program.
The position is hybrid, allowing for up to 3 in-person days and 2 remote days.
Coordinate care and clinically manage patients and caregivers.
Work schedule consists of five 8-hour days per week.
No weekends, nights, on-call, or holidays required.
Duties include patient education and reviewing after visit summaries.
Manage EPIC in-basket, triage calls, and return messages to families.
Write letters and outreach to patients regarding barriers to care.
Coordinate with DME, nursing agencies, and other vendors.
Review patient charts as needed.
Medication reconciliation or refills will not be part of this role.
