Telehealth RN: Case Manager & Care Coordinator

Veterans Health Administration seeks a Registered Nurse - Home Telehealth Case Manager to coordinate care across inpatient and outpatient settings, guiding education, self-management, and patient satisfaction while leading the nursing process.
