Job Summary

Responsibilities

Key Responsibilities

Patient Assessment:
Conduct comprehensive geriatric assessments, physical exams, and diagnose complex, multiple health issues common in aging.
Treatment Planning:
Develop personalized care plans addressing unique medical, psychological, and social needs, often involving multiple chronic diseases.
Medication Management:
Carefully prescribe and monitor medications, focusing on preventing adverse interactions common in older adults.
Care Coordination:
Collaborate with nurses, therapists, social workers, and other specialists for holistic, integrated care.
Patient & Family

Education:

Provide health education, lifestyle advice, and support to patients, families, and caregivers.
Preventative Care:
Offer health screenings, vaccinations, and guidance for healthy aging and disease prevention.
Supportive Care:
Manage pain, mobility issues, and provide end-of-life/palliative care counseling.

Training:
Qualified physician (often internal medicine or family medicine) with additional geriatric training.
