Location: Bangor
Ysbyty Gwynedd, North Wales, is recruiting a Band 6 Nurse Practitioner to join the Psychiatric Liaison Older Person Team. The role supports a 24/7 assessment service for acute hospital in-patients, coordinating mental health assessments and follow-up within a multidisciplinary setting.
The post holder will triage over-70s accessing ED, perform risk assessments, and develop safe discharge plans while liaising with Primary, Secondary and Community services. Welsh language ability is desirable.
