Location: Bangor

Responsibilities

  • Work within an established, integrated, multidisciplinary team, focusing on individuals with severe and enduring mental health problems.
  • Undertake responsibility for the care co‑ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.
  • Facilitate promotion and maintain a high standard of nursing care adhering to current legislation, professional and corporate guidelines.
  • Provide specialist assessment, treatment and care to a caseload of people with severe and enduring mental health difficulties from within the local community.
  • Provide care to service users within the legal frameworks of the Mental Health (Wales) Measure 2010, the Social Services and Well‑Being (Wales) Act 2014 and the Mental Health Act (1983).
Qualifications
  • Must be enthusiastic, self‑motivated, flexible in approach, able to work autonomously as well as with statutory and non‑statutory service providers.
  • Effective communication and engagement skills are fundamental to this role.
  • Current evidence‑based knowledge and a commitment to working with individuals adopting a recovery approach and a trauma‑informed model of care.
