** Position Summary
** Impact Health PC is seeking a
** Psychiatric Mental Health Nurse Practitioner (PMHNP)
** to provide psychiatric care to residents within
** Skilled Nursing Facilities in the Bangor, ME area**.You will assess, diagnose, and manage psychiatric and behavioral health conditions, develop individualized treatment plans, and collaborate with physicians, facility staff, and interdisciplinary care teams to ensure high-quality patient care.###
** Key Responsibilities
*** Provide comprehensive
** psychiatric evaluations and ongoing medication management**.
* Assess, diagnose, and treat psychiatric and behavioral health conditions.
* Develop evidence-based, individualized treatment plans.

* Provide patient and family education regarding diagnoses, medications, and treatment plans.

** Qualifications* Active
** Psychiatric Mental Health Nurse Practitioner (PMHNP) license/certification in Maine**, as applicable.
* Minimum
** 2 years of clinical experience
** providing mental health care to adult populations.
* Experience in
** Skilled Nursing, Long-Term Care, Geriatrics, Hospital, Residential Care, or Outpatient Psychiatry
** preferred.
* Strong psychiatric assessment, diagnostic, and medication management skills.
* Proficiency with electronic health records (EHRs) and clinical documentation.
* Strong communication, organizational, and collaborative skills.
* Ability to practice independently within applicable state regulations while working closely with an interdisciplinary care team.
